EAP visits are generally paid for by your employer. Generally speaking, 
an employer pays for 2-4 visits per year. It is their "gift" to you to assist
you with personal problems that may be interfering with your work. It is used
as a short-term assessment/evaluation.

Program Description for ASAM Level I Outpatient Substance Abuse Treatment & Services:
Circle of Life Center will be providing ASAM Level-I Outpatient Substance Abuse Treatment
and Education Services. Level I encompass organized outpatient treatment services, 
which may be delivered in a wide variety of settings. In Level I programs, addiction
treatment staff, including addiction-credential counselors, provide professional directed
evaluations, treatment and recovery services. Such services are provided in regularly scheduled
sessions of (usually) fewer than nine contact hours a week. The services follow a defined set 
of policies and procedures or clinical protocols. 

ASAM Level I Services are tailored to each client’s level of clinical severity and are
designed to help the client achieve changes in his or her alcohol or other drug using behaviors.
Treatment thus must address major lifestyle, attitudinal and behavioral issues that have the 
potential to undermine the goals of treatment or to impair the individual’s ability to cope
with major life tasks without the non-medical use of alcohol or other drugs. The duration of 
treatment varies with the severity of the individual’s illness and his or her response to treatment.


Essential Features of ASAM Level I Outpatient Substance Abuse Treatment and Services: 

	ASAM Level-I services may be delivered in office practice, health clinics,
	 primary care clinics addiction and mental health clinics.

	Setting- ASAM Level-I programs services may be offered in any appropriate 
	setting that meets state licensure or certification criteria.  

	Support Systems- In ASAM Level-I programs, necessary support systems include:

	Medical, psychiatric, psychological, laboratory and toxicology services, 
	which are available on-site or through consultation or referral. Medical and
	 Psychiatric consultations are available within 24 hours by telephone and/ or
	 if in person within a time frame appropriate to the severity and urgency of 
	the Consultation requested.

	Direct affiliation with or close coordination through referral to more 
	intensive Levels of care and medication management.

	Emergency services available by telephone 24 hours a day, 7 days a week.    


DOT-SAP- Return to Duty Services Include:

Initial assessment which includes a standardized test with proven testing tools 
which assists in determining the severity of the addiction, how motivated the
patient is, how to best treat the problem, and how likely relapse is.
 
The SAP recommendations to the client for the most appropriate treatment provider(s)
in their geographic location. 

An initial letter to be sent to the employer delineating the SAP's recommendation(s)
and the recourse(s) available. 

The SAP requires a Consent for Disclosure to be signed by the employee in order that
the SAP may check the employee's compliance with the recommended treatment provider. 

In addition, the SAP requires that the employee contact the SAP with a treatment 
status on a weekly basis. A final session with the employee will be conducted upon 
completion of the recommended treatment.
 
A follow-up letter will be sent to the employer stating whether or not the employee
has successfully followed and completed the treatment recommendation(s) originally 
set forth by the SAP.

Copperfield Counseling & Psychotherapy is a small counseling & psychotherapy run 
by a single practitioner. Psychotherapist Sherell Hebert, LPC is licensed by the
